I find the chorus to be just a filler, because if you read the lyrics between the repeated chorus, it really tells a story. This is my interpretation of it. The first two paragraphs, in my opinion, are about his lover being in the hospital, in a hospital bed.

Apparantly eddy wrote this song for lou because of lou's past relationship with hannah louhannah and that's why lou's part is emotional, it's said that it's about how louis wanted hannah back, but no one knows the truth c: and since ed is friends with 1d he could've got the idea from them, but then again there's a rumor it's about a girl who dies from some disease and her boyfriend committing suicide because he couldn't live without her.
